Each of our members is assigned a “Success Team”, which encompasses a dedicated social worker and case manager. Members begin their Empower Nation journey by crafting a 5yr plan with their Success Team. This plan is carefully segmented into 1 month, 3 month, 6 month, 1 year, 3 year and 5 year goals.

Members are then provided with a virtual appointment scheduled, from which they video conference with their Success Team on a weekly basis. These weekly meetings allow us to support our members on their 5yr journey.

Our Success Team’s serve as not just social workers and case managers, but coaches, mentors, care coordinators, guidance counselors and much more. This team does the often arduous work of enrolling members in programs to support their needs such as housing, food, education, transportation, clothing and more.

Each journey begins with a focus on the member’s basic needs, it quickly progresses into career advancement, entrepreneurship, community engagement, and continuing education.

Our social workers and case managers follow evidence based practices, specifically the Empowerment Theory, which uses intervention methods to help guide people toward building autonomy and control in their own lives. All of these success management services are provided to our members at no cost!
